Distracteur à appui osseux versus distracteur à appui dentaire dans la distraction orthodontique après disjonction maxillaire chirurgicale : l’avis du patient

Résumé

Introduction: Orthodontic distraction after surgical maxillary expansion is a mode of treatment regularly used in the context of transverse maxillary constriction. There is, however, no consensus in the literature as to the type of distractor (bone-borne or tooth-borne) that should be used. Patients and method: This retrospective study compared orthodontic distraction using a bone-borne or a tooth-borne distractor from the point of view of tolerance, ease of use and overall patient satisfaction, by means of a questionnaire completed by patients undergoing maxillary expansion surgery in the Stomatology and Maxillofacial Surgery Department of Lille University Hospital between January 2013 and March 2015. The efficacy of the two distractors was also assessed. Results: Thirty-two patients were included: 10 in the bone-borne distractor group and 22 in the tooth-borne group. Sixty percent of patients questioned found the bone-borne distractor easy to use compared with 32% for the tooth-borne distractor (P = 0.167). Tolerance was noted to be comparable and acceptable by the two groups. The overall satisfaction rate was high for both groups at over 90%, and was correlated with ease of use and clear information. The average space gain between the first molars was 11.1 mm with the bone-borne device and 10.7 mm for the tooth-borne appliance Conclusion: The use of a bone-borne distractor for orthodontic distraction after maxillary expansion surgery appears to be an effective, simple and well-tolerated alternative to the use of a tooth-borne distractor. According to patients, this distractor also appears easier to use than the traditional Hyrax-type distractor. This ease of use is correlated with overall satisfaction.
Introduction La distraction orthodontique après disjonction intermaxillaire chirurgicale est un traitement couramment utilisé dans le cadre des insuffisances transversales maxillaires. Cependant, il n’existe pas de consensus dans la littérature sur le type de distracteur (appui osseux ou appui dentaire) à utiliser. Patients et méthode Cette étude rétrospective a comparé la distraction orthodontique par distracteur à appui osseux versus distracteur à appui dentaire du point de vue de la tolérance, de la facilité d’utilisation ainsi que de la satisfaction globale du patient par questionnaire chez les patients opérés d’une disjonction intermaxillaire dans le service de stomatologie et chirurgie maxillofaciale du CHRU de Lille entre janvier 2013 et mars 2015. L’efficacité de ces deux distracteurs a également été évaluée. Résultats Trente-deux patients ont été inclus : 10 dans le groupe distracteur à appui osseux et 22 dans le groupe distracteur à appui dentaire. Au total, 60 % des patients interrogés ont trouvé le distracteur à appui osseux de facile à très facile d’utilisation contre 32 % pour le distracteur à appui dentaire (p = 0,167). La tolérance a été trouvée comparable et acceptable pour les deux groupes. Le taux de satisfaction globale était élevé pour les deux groupes à plus de 90 %, et corrélé à la facilité d’utilisation et à une information claire. Le gain de place moyen entre les premières molaires a été de 11,1 mm pour l’appui osseux contre 10,7 mm pour l’appui dentaire. Conclusion L’utilisation d’un distracteur à appui osseux dans le cadre de la distraction orthodontique après disjonction intermaxillaire chirurgicale apparaît être une alternative efficace, simple et tolérable, à l’utilisation des disjoncteurs à appui dentaire. Selon l’avis des patients, ce distracteur semble également plus facile d’utilisation que le distracteur traditionnel de type hyrax ; facilité d’utilisation qui est corrélée à la satisfaction globale.
